Infective endocarditis (IE) is life threatening infection of endocardial inner surface caused by bacterial, fungal and viral. Predisposition mostly with congenital heart defect with poor oral hygiene or surgical procedure. We report 7 IE cases in children from 2016 to April 2018 with 2 cases of Tetralogy of Fallot, 1 case Patent Ductus Arteriosus, 1 case with HIV, and 3 cases post rheumatic heart disease. Demographically ages between 6 - 17 years old. Major compliment was high fever, fatigue, palpitation. We found vegetation in most cases, but in HIV case we found vascular and immune phenomenon instead. All patient received empiric antibiotic therapy. HIV and 1 RHD case pass away during therapy due to pulmonary embolism and cardiac failure. Only 2 blood culture was positive in all 7 cases. Underlying disease can be cardiovascular or non-cardiovascular disease. Mostly blood culture was negative. Immunology and vascular phenomenon in patient can be a sign of bad prognosis
