Perbandingan skor NOSE (Nasal Obstruction Symptom Evaluation) pada pasien deviasi septum sebelum dan sesudah pembedahan di RSUP Sanglah Denpasar tahun 2018

Abstract

Background: Nasal congestion is a nasal complaint that most often makes people come for treatment, where septal deviation is the most common cause. The septum abnormality in the form of septal deviation has a significant value because it is quite influential in most of the rhinological problems. This study aims to determine the comparison of NOSE (Nasal Obstruction Symptom Evaluation) scores in patients with septal deviation, before and after surgery at Sanglah General Hospital Denpasar in 2018.

Method: This study is an observational study of the one-group test posttest design (before-after study). The study sample was all patients with septal deviation who underwent surgery at Sanglah Hospital Denpasar in 2018 who met the inclusion and exclusion criteria.

Result: From 43 research subjects, there were 27 male or 62.8% and 16 female or 37.2%. On the age characteristics, the mean age of the subjects was 39.77 years with SD value ± 14.673 years, where the youngest age was 16 years, and the oldest was 75 years. Based on the characteristics of accompanying diagnoses, the highest number was concha hypertrophy of 33 (76.7%) and the majority of surgeries performed were performed by submucosal resection techniques which were 39 (90.7%). The degree of complaints of nasal obstruction based on NOSE before surgery majority complained of moderate obstruction, with an average NOSE score of 44.77 and a standard deviation of 10.519. The degree of complaint of nasal obstruction based on NOSE after the majority of surgery does not complain about nasal obstruction, with an average NOSE score of 2.91 and a standard deviation of 4.785. The different NOSE scores before and after surgery were 41.86 with p = 0.000.

Conclusion: there is a difference between NOSE scores in patients with septal deviation, before and after surgery at Sanglah General Hospital Denpasar.


Pendahuluan: Hidung tersumbat merupakan keluhan hidung yang paling sering membuat orang datang berobat, dimana deviasi septum nasi adalah penyebab yang tersering. Kelainan septum berupa deviasi septum nasi memiliki nilai yang signifikan karena cukup berpengaruh terhadap sebagian besar permasalahan rinologis. Penelitian ini bertujuan untuk mengetahui perbandingan skor NOSE (Nasal Obstruction Symptom Evaluation) pada pasien deviasi septum, sebelum dan sesudah pembedahan di RSUP Sanglah Denpasar tahun 2018.

Bahan dan metode: Penelitian ini merupakan studi observasional the one-group test posttest design (before-after study). Sampel penelitian adalah semua penderita dengan deviasi septum yang menjalani pembedahan di RSUP Sanglah Denpasar tahun 2018 yang memenuhi kriteria inklusi dan eksklusi.

Hasil: Dari 43 subjek penelitian didapatkan jenis kelamin laki-laki sebanyak 27 penderita atau 62,8% dan perempuan 16 penderita atau 37,2%. Pada karakteristik umur didapatkan rerata umur subjek adalah 39,77 tahun dengan nilai SD ± 14,693 tahun, dimana umur subjek termuda adalah 16 tahun, dan tertua 75 tahun. Berdasarkan karakteristik diagnosis penyerta, didapatkan terbanyak adalah hipertrofi konka sebanyak 33 (76,7%) dan pembedahan yang dilakukan mayoritas dilakukan dengan teknik reseksi submukosa yaitu 39 (90,7%). Derajat keluhan sumbatan hidung berdasarkan NOSE sebelum pembedahan mayoritas mengeluhkan sumbatan sedang, dengan rerata skor NOSE 44,77 dan standar deviasi 10,519. Derajat keluhan sumbatan hidung berdasarkan NOSE sesudah tindakan pembedahan mayoritas tidak mengeluhkan sumbatan hidung, dengan rerata skor NOSE 2,91 dan standar deviasi 4,785. Beda rerata skor NOSE sebelum dan sesudah pembedahan didapatkan sebesar 41,86 dengan nilai p = 0,000.

Kesimpulan: terdapat perbedaan antara skor NOSE pada pasien dengan deviasi septum, sebelum dan sesudah menjalani pembedahan di RSUP Sanglah Denpasar.
