Evaluasi radiologi pada trauma tembus kompleks nasoorbitoethmoid

Abstract

Nasoorbitoethmoid (NOE) complex defined as structure that surrounds important fine structure, the nasolakrimal drainage system and medial canthal ligament. It also contiguous with vital region including the orbit and the anterior cranial fossa so does the structure in it. Although the penetrating injuries of the NOE complex is rare, but it provides complicated treatment and sequels due to its location and the structure within it. We reported a case of penetrating injuries at the NOE complex in a 28 years old mal patient. The patient complains of pain in both eyes followed by a blurred vision on the left eye. Axial head CT scan showed smooth and firm edge corpus alienum with air density (-178 to -955 HU) got through the medial aspect of the left eye and emerge into left ethmoidale sinus, os vomer and the tip end at right ethmoidale sinus. The patient undergone surgical treatment to remove the corpus alienum and get good result after surgery.



Kompleks nasoorbithoethmoid (NOE) merupakan struktur yang melingkupi sekelompok struktur halus yang penting seperti sistem drainase nasolakrimal dan ligamen kantus medial. Struktur ini juga berhubungan dengan daerah penting lainnya termasuk orbita dan fossa kranial anterior serta struktur di dalamnya. Walaupun trauma tembus pada kompleks NOE jarang terjadi, tetapi penanganan dan gejala sisa yang ditimbulkan cukup kompleks akibat lokasi dan struktur yang terdapat di dalamnya. Kami melaporkan sebuah kasus trauma tembus kompleks NOE pada pasien lelaki berusia 28 tahun. Pasien mengeluh nyeri pada kedua bola mata yang diikuti dengan pandangan mata kiri yang semakin kabur. CT scan kepala potongan aksial menunjukkan benda asing berdensitas udara (-178 sampai dengan -955 HU) dengan tepi licin dan batas tegas yang memasuki aspek medial mata kiri dan menembus sinus ethmoidalis kiri, os nasal, dan berakhir di sinus ethmoidalis kanan. Pasien menjalani tindakan pembedahan untuk mengangkat benda asing tersebut dan mendapatkan hasil yang cukup baik setelah tindakan pembedahan.
