Satu kasus linear IgA dermatosis pada anak

Abstract

Background: Linear immunoglobulin A (IgA) dermatosis is a rare immunized mediated disease, characterized by homogeneous linear IgA deposition of the basal membrane of the skin. This case occurs most often after puberty, with most patients seen after the fourth decade with a clinical picture of tense bullae with symmetrical spread on extensors including elbows, knees and buttocks accompanied by itching. Reportedly this case is more common in Europeans and rarely in Asians and Afroamerica, prevalence around 1.2-39.2 per 100,000 children.The therapy given is dapsone but if the intolerant patient can dapone given systemic glucocorticoids as a single therapy.

Case Ilustration: In this case report reported cases of Linear immunoglobulin A (IgA) dermatosis on the face, body and hands in 11 year old girls. Diagnosis is based on history, physical examination and histopathological examination. Patients in this case report were given systemic methylprednisolone glycocorticoid therapy. Patients showed good results after therapy, and no side effects occurred during treatment.

Conclusion: The case prognosis was dubius ad bonam.


Latar Belakang: Linear immunoglobulin A (IgA) dermatosis merupakan penyakit bula yang dimediasi imun yang jarang, ditandai dengan deposisi IgA linear homogen pada membran basal kulit. Kasus ini terjadi paling sering setelah pubertas, dengan sebagian besar pasien tampak setelah dekade keempat dengan gambaran klinis berupa bula tegang dengan penyebaran yang simetris pada ekstensor termasuk siku,lutut dan bokong disertai rasa gatal.Dilaporkan kasus ini lebih sering terjadi pada orang Eropa dan jarang pada orang Asia dan Afroamerika, prevelensi sekitar 1,2-39,2 per 100.000 anak. Terapi yang diberikan adalah dapsone tetapi apabila pasien intoleranterhadap dapson dapat diberikan glukokortikoid sistemik sebagai terapi tunggal.

Ilustrasi Kasus: Pada laporan kasus ini dilaporkan kasus Linear immunoglobulin A (IgA) dermatosis pada wajah,badan dan kedua tangan pada anak perempuan usia 11 tahun.Diagnosis ditegakkan berdasarkan anamnesis, pemeriksaan fisik dan pemeriksaan histopatologi. Pasien pada laporan kasus ini diberikan terapi glikokortikoid sistemik metilprednisolon.. Pada pasien menunjukkan hasil baik paska terapi, dan tidak terjadi efek samping selama pengobatan.

Kesimpulan: Prognosis pada pasien dubius ad bonam.
