Manajemen anestesi pada pasien pediatri yang menderita atresia duodenum dengan penyakit penyerta atrioventicular septal defect komplit (atrial septal defect primum, single ventrikel) dengan malposisi dari arteri besar disertai dengan pulmonal stenosis mod

Abstract

Atrioventricular septal defect (AVSD) covers a spectrum of heart anomalies with a common atrioventricular connection and has an incidence of 4–5.3 per 10.000 live births. In atrioventricular septal defect, the common AV valve can have a common orifice or can be divided by bridging leaflet tissue into two separate orifice. Congenital intrinsic duodenal obstruction may be caused by duodenal atresia, stenosis, membrane, or web and most frequently occurs in the second part of the duodenum at or below the ampulla of Vater. For the surgical treatment of congenital intrinsic duodenal obstruction KIMURA, in 1977 introduced an anastomotic technique of side-to-side duodenoduodenostomy in two layers, arranging the bowel incisions to form a “diamond-shaped†and created a larger stoma.

 

Atrioventricular Septal Defect (AVSD) merupakan kelainan jantung dimana adanya hubungan (kebocoran) antara ruang atrium jantung dan ruang ventrikel jantung yang umum dan memiliki insiden 4-5,3 per 10.000 kelahiran hidup. Pada defek septum atrioventrikular, katup AV umumnya dapat mempunyai satu kebocoran atau dapat dibagi menjadi beberapa kebocoran yang terpisah. Obstruksi duodenum bawaan mungkin disebabkan oleh atresia duodenum, stenosis, membran, atau jaringan dan paling sering terjadi pada bagian kedua dari duodenum atau di bawah dari ampula Vater. Untuk terapi pembedahan pada obstruksi duodenum intrinsik bawaan diperkenalkan pertama kali  oleh KIMURA pada tahun 1977 yaitu teknik anastomotik dimana dilakukan side-to-side duodenoduodenostomi dalam dua lapisan, mengatur sayatan usus untuk membentuk "diamond-shaped" dan menciptakan stoma yang lebih besar.
