Hiperplasia sebasea yang diterapi dengan bedah beku

Abstract

Hiperplasia kelenjar sebasea adalah manifestasi tumor kulit jinak pada wajah berupa papul, sewarna kulit atau kekuningan. Pada kasus ini, metode pengobatan seperticryosurgeryatau bedah beku dapat digunakan, dengan mempertimbangkan jumlah lesi dan harapan kosmetik pasien. Seorang perempuan, 28 tahun, datang dengan keluhan utama permukaan kulit hidung yang tidak rata sejak 5 tahun. Bedah beku adalah metode dimana terjadi pendinginan jaringan dengan temperatur ekstrim, dan membiarkan jaringan mencair secara perlahan. Kerusakan jaringan disebabkan oleh kristalisasi cairan dalam sel dan iskemia pemuluh darah yang disebabkan oleh trombosis. Prosedur non-invasif ini cenderung tidak sulit untuk dilakukan, cepat dan memiliki tingkat komplikasi yang rendah, sehingga metode ini dipilih sebagai terapi pada laporan kasus ini. Nitrogen cair merupakan kriogenpilihan dalam bidang dermatologi.Nitrogen  cair mudahuntukdisimpan dan digunakan, ramah lingkungan, tidak mudah terbakar, harga yang terjangkau, sertamemiliki suhu terendah sehingga menimbulkan pembekuan yang cepat pada jaringan yang diterapi. 3 hari paska tindakan, luka bedah beku menjadi bula, lalu pecah meninggalkan erosi. Penangganan selanjutnya, diberikan kompres normal saline tiap 8 jam selama 15 menit, dilanjutkan dengan krim antibiotik natrium fusidat tiap 12 jam. Pasien kontrol hari ke 14 paska tindakan, luka bedah beku membaik. Tekstur kulit tampak lebih rata.

 

Sebaceous gland hyperplasia is a manifestation of benign skin tumors on the face in the form of papules, yellowish or skin-coloured lesion. In this case, treatment method of cryosurgery was used, taking into account the number of lesions and the patient’s cosmetic goal. A woman, 28 years old, came with the chief complaint of an uneven skin bump on the nose for 5 years. Cryosurgery is a method, in which the skin tissue is being cooled down to extreme temperatures, and being allowed for the tissue to thaw slowly. Tissue damage was caused by crystallization of fluid inside the cells and blood vessel thrombotic ischemia. This non-invasive procedure tends to be easy to perform, fast and has a low complication rate. Therefore, this method was chosen as the therapy in this case report. Liquid nitrogen is the preferred cryogen in the field of dermatology. Liquid nitrogen is easy to store and use, environmentally friendly, non-flammable, affordable prices, and has the lowest temperature causing rapid freezing of treated tissue. 3 days after the procedure, the surgical wound turned into bullae, which then breaks off, leaving an erosion. Patient was given normal saline compresses every 8 hours for 15 minutes, followed by sodium fusidate antibiotic cream every 12 hours. Patients controlled for day 14 after the procedure, and the surgical wounds improved. Skin texture became smoother
