Koagulopati Dini sebagai Faktor Resiko Mortalitas pada Trauma Abdomen di Rumah Sakit Sanglah Periode Tahun 2015-2016

Abstract

Latar Belakang: Data yang didapatkan dari Rumah Sakit Sanglah tercatat pada tahun 2015 menyatakan bahwa dari total 2755 tindakan di ruang operasi IRD RS Sanglah, didapatkan 720 kasus cedera kepala, 455 dengan fraktur ekstremitas dan 64 kasus dengan trauma abdomen, sisanya berkaitan dengan kegawatdaruratan bedah non trauma. Tujuan: Tujuan penelitian ini adalah untuk mengetahui pengaruh profil koagulasi dini yaitu pemanjangan  Prothrombin Time (PT) dan Activated Partial Thromboplastin Time (APTT)  pada pasien dengan trauma abdomen terhadap terjadinya mortalitas di Rumah Sakit Umum Pusat Sanglah. Metode: Penelitian ini menggunakan rancangan case control. Hasil: Hasil analisis menunjukkan bahwa faktor risiko PT pada kelompok yang meninggal semuanya atau 100,0% (17 orang) mengalami pemanjangan PT, sedangkan pada kelompok yang hidup didapatkan sebanyak 68% (35 orang) yang mengalami pemanjangan PT. Perbedaan proporsi tersebut menunjukkan pemanjangan PT merupakan faktor risiko terjadinya mortalitas yang secara statistik bermakna (p=0.007). Faktor risiko APTT pada kelompok yang meninggal sebagian besar atau 70,6% (12 orang) mengalami pemanjangan APTT, sedangkan pada kelompok yang hidup didapatkan sebagian kecil atau 23,5% (12 orang) yang mengalami pemanjangan APTT. Perbedaan proporsi tersebut menunjukkan pemanjangan APTT merupakan faktor risiko terjadinya mortalitas dengan Odds Ratio 7,8 dan  secara statistik bermakna (p<0,001). Odds ratio 7,8 menunjukkan bahwa resiko APTT memanjang untuk mengalami mortalitas 7,8 kali dibandingkan dengan yang tidak memanjang. Kesimpulan: Pemanjangan PT > 14,4 dan atau APTT > 36 terbukti secara statistik sebagai faktor risiko terjadinya mortalitas pasien trauma abdomen di RSUP Sanglah Denpasar.
