Hubungan antara aktivitas penyakit dan kualitas hidup pada lupus eritematosus sistemik pediatrik

Abstract

Background: Pediatric systemic lupus erythematosus (pediatric SLE) is a systemic autoimmune disorder that results in fluctuating chronic inflammation and morbidity. Therefore, the nature of the disease and long-term therapy may impact quality of life. This study aims to analyze the relationship between disease activity and quality of life in children with SLE.

Method: Retrospective cross-sectional study of children suffering from SLE at Prof.Dr.IGNG Ngoerah General Hospital, Denpasar, 2015-2021. Quality of life was assessed using PedsQL and SLE disease activity was assessed using the SLEDAI score. Statistical analysis consisted of correlation tests, Kruskal-Wallis followed by ANOVA and regression analysis.

Results: The subjects of this study were 85 people with a median age of 14 years with a female to male ratio of 3:1. Most subjects experienced mild SLE, namely 33 people (38.8%) and were involved in light activity. The mean PedsQL of children and parents was 70.6 (17.8%) and 71.6 (15.75%), respectively. PedsQL of children and parents had a significant correlation with disease activity (R:-0.485, p<0.001) and (R:-0.509, p<0.001). The Kruskal-Wallis test showed a significant relationship between the quality of life of children and parents and the degree of SLEDAI score (p<0.001). ANOVA test showed initial therapy, nutritional status, SLEDAI score, age, anemia, lymphopenia, neutropenia and disease duration were significant predictors for PedsQL for children and parents (F:5.42 and 4.265, p<0.001). The linear regression test showed that the SLEDAI score was the only significant predictor of quality of life (B:-1.203 and -1.181, 95%CI -1.686, 0.721 and -1.667, -0.694, p<0.001).

Conclusion: There is a significant relationship between disease activity and the quality of life of children and parents. The SLEDAI score is a significant predictor of quality of life.

 

Latar Belakang: Lupus eritematosus sistemik anak (SLE pediatrik) mengalami kelainan autoimun sistemik yang mengakibatkan fluktuasi inflamasi kronik dan morbiditas. Oleh karena itu, sifat penyakit dan terapi jangka panjang mungkin berdampak pada kualitas hidup. Penelitian ini bertujuan untuk menganalisis hubungan aktivitas penyakit dengan kualitas hidup pada SLE pediatrik.

Metode: Studi cross-sectional retrospektif pada anak penderita SLE di RSUP Prof.Dr.IGNG Ngoerah Denpasar tahun 2015-2021. Kualitas hidup dinilai menggunakan PedsQL dan aktivitas penyakit SLE dinilai menggunakan skor SLEDAI. Analisis statistik terdiri dari uji korelasi, Kruskal-Wallis dilanjutkan dengan ANOVA dan analisis regresi.

Hasil: Subjek penelitian ini berjumlah 85 orang dengan median usia 14 tahun dengan rasio perempuan dan laki-laki 3:1. Subjek terbanyak mengalami SLE ringan, yaitu 33 orang (38,8%) dan berada dalam aktivitas ringan. Rerata PedsQL anak dan orang tua masing-masing sebesar 70,6 (17,8%) dan 71,6 (15,75%). PedsQL anak dan orang tua mempunyai korelasi yang signifikan terhadap aktivitas penyakit (R:-0,485, p<0,001) dan (R:-0,509, p<0,001). Uji Kruskal-Wallis menunjukkan hubungan yang signifikan antara kualitas hidup anak dan orang tua dengan derajat skor SLEDAI (p<0,001). Uji ANOVA menunjukkan terapi awal, status gizi, skor SLEDAI, usia, anemia , limfopenia, neutropenia dan durasi penyakit merupakan prediktor signifikan untuk PedsQL anak dan orang tua (F:5,42 dan 4,265, P-value <0,001). Uji regresi linier menunjukkan bahwa skor SLEDAI merupakan satu-satunya prediktor yang signifikan terhadap kualitas hidup (B:-1,203 dan -1.181, 95%CI -1,686, 0,721 dan -1,667, -0.694, p<0,001).

Kesimpulan: Terdapat hubungan yang signifikan antara aktivitas penyakit dengan kualitas hidup anak dan orang tua. Skor SLEDAI merupakan prediktor yang signifikan terhadap kualitas hidup.
