Gelembung udara besar pada abdomen neonatus: Sebuah kasus kista koledokus

Abstract

Choledochal cyst are rare cystic dilatations of the biliary tree. A baby girl, 0 days with an enlarged abdomen, palpable mass and jaundice from birth. Laboratory investigations have shown improvement in liver function. X-rays, abdominal ultrasonography and CT scan revealed type I choledochal cyst. Biopsy result shown supporting the diagnosis of choledochal cysts with fibrotic tissue in the port area. Cyst excision and biliary repair have been performed with the patient's clinical improvement. Choledochal cyst were usually diagnosed in childhood with first decade of life. This abnormality primarily affects girls. In addition to this, radiological examination guide for a detailed examination in order to verify the diagnosis. In view of the high risk of cholangiocarcinoma, early diagnose of the choledochal cyst had an important rule to decrease morbidity and mortality through radiological imaging techniques.

 

Kista koledokus adalah dilatasi kistik yang jarang dari sistem bilier. Seorang bayi perempuan, 0 hari dengan perut membesar, teraba massa dan ikterus sejak lahir. Pemeriksaan laboratorium menunjukkan peningkatan fungsi hati. Sinar-X, ultrasonografi dan CT scan abdomen menunjukkan dilatasi duktus koledokus tipe I. Hasil biopsi mendukung diagnosis kista koledokus dengan jaringan fibrotik di daerah porta. Eksisi kista dan rekonstruksi bilier telah dilakukan dengan hasil perbaikan klinis pasien. Kista koledokus biasanya didiagnosis pada dekade pertama kehidupan. Kelainan kongenital ini terutama menyerang anak perempuan. Selain itu, pemeriksaan radiologi dapat membantu untuk pemeriksaan secara rinci untuk menegakkan diagnosis. Mengingat tingginya risiko kolangiokarsinoma, diagnosis dini kista koledokus memiliki peranan penting untuk mengurangi morbiditas dan mortalitas melalui teknik pencitraan radiologi.
